How to build your FMEA:

1 - Scope the work and build the team.

Define the process.
Bring in quality, engineering, and operations.

2 - List failures, effects, and causes.

Ask what each step should do.

Then ask:

What could fail?
What happens if it fails?
Why would it happen?

3 - Rate the risk.

Severity: How bad is the impact?
Occurrence: How often could it happen?
Detection: Can we catch it in time?

4 - Calculate the RPN.

RPN = Severity × Occurrence × Detection

Higher RPN means higher risk.

Fix those items first.

5 - Plan actions.

Add controls
Assign owners
Set due dates
Focus on high-risk items first

6 - Re-score and track.

Keep it alive as the process changes.
